We begin with a deep dive into your symptoms, health history, and lifestyle so I can understand what’s going on beneath the surface.
✅ Standard Lab Assessment
Have recent labs (within the past 6 months)? I’ll review them through a functional lens—looking beyond "normal" to find what’s optimal for you.
✅ Your Personalized Wellness Roadmap
You’ll leave with a practical, holistic plan tailored to your unique biology, goals, and lifestyle—including nutrition, stress support, and more.

With a blend of functional medicine, holistic therapies, and lifestyle strategies, we’ll address the root causes of your symptoms rather than simply masking them. Together, we begin mapping out the foundations of your personalized Wellness Map, using the D.R.E.S.S. Framework (Diet, Rest, Exercise, Stress, Supplementation) to guide our understanding.

🌺 Before We Meet…
Once you’ve booked your session, you’ll receive The Metabolic Scorecard. My review of your answers will guide us toward your Personalized Wellness Roadmap.
You will also complete the Lifestyle History Form in the client portal.
Allow for 45-90 minutes to complete both. Assessments must be completed and sent back no later than 3 business days before the scheduled session (schedule accordingly.)
This is a very in debth, comprehensive assessment of you, requiring honest, truthful answers to the assessment, so that I can connect the dots and provide the best roadmap for you.
By the end of your Rooted Wellness Assessment, you’ll walk away with:
